File of Life

A File of Life document consists of an individual’s emergency contact and medical information. The file serves the following purposes:

  • Allow individuals to prepare contact and medical information prior to embarking on any cruise.
  • Allow individuals to provide personal contact and medical information to the skipper in a confidential form.
  • To have on board emergency contact and medical information for all individuals.
Two Pelagic Sailing Club versions are available for download:

  • Single Page:   [PDF]    1. Medical & Contact Information
  • Double Page: [PDF]    1. Medical & Contact Information    2. Name

Instructions for all individuals: (skippers, crew, members, applicants, passengers, guests):

  1. Fill out all medical and contact information on your File of Life form(s) well in advance of any cruise. Securely enclose a completed single page form in an envelope labeled with your name. Or, fold a completed double page form in a manner that displays only your name.
  2. Prior to embarking on a cruise, present your form to the skipper for safe keeping during the entire cruise. Note where everyone’s File of Life forms (and all other emergency gear) are located on board.
    1. Only in the event of an emergency: obtain the victim’s form and present the information as directed to the Coast Guard or First Responders.
    2. Otherwise: regard all File of Life forms and information as personal and confidential.
  3. Retrieve your own form from the skipper prior to disembarking from the vessel at the end of the cruise.

Instructions for skippers:

  1. Prepare your own individual File of Life form(s) as per steps 1 and 2 of the above ‘Instructions for All Individuals’.
  2. Prior to embarking on a cruise:
    1. Collect all File of Life form(s) from all who will be on board during the cruise. Secure all forms in a waterproof bag, container, or compartment.
    2. Show to all on board where everyone’s File of Life forms (and all other emergency gear) are located. Brief everyone on how the forms (and all other emergency gear) are used, especially if the skipper is incapacitated.
    1. Only in the event of an emergency: obtain the victim’s form and present the information as directed to the Coast Guard or First Responders.
    2. Otherwise: regard all File of Life forms and information as personal and confidential.
  3. Return each form to their owners just before they disembark from the vessel at their end of the cruise.

One Response to Forms

  1. Alex Collier says:

    Hello Pelagic Sailing Club
I have heard about your club and would like to come to the April 27 meeting.
Can my wife and I just show up?

I have a Beneteau Oceanis 461 currently at Wickford Marina, RI and frequently looking for crew to join me on a sail.

    Alex
